We're building an app that helps patients — starting with those managing heart failure and other chronic conditions — track daily weight, blood pressure, and other key signals at home, with alerts when something trends dangerous. That's one piece of a bigger goal: helping people stay independent, at home, and out of the hospital as much as possible.
